Form 1040B - IRS - must be completed by U.S. citizens and residents who have a foreign trust or a foreign financial account.
Form 114 - Treasury - exceeding certain thresholds, the Bank Secrecy Act may require you to report the account yearly to the Internal Revenue Service.
